I hereby grant permission for my child/children to participate in Sole Soccer Camp being held at Mississauga Stadium in Mississauga, Ontario, Canada from December 27-29, 2019. I fully understand and acknowledge that risks and dangers exist in my child/children’s involvement at Sole Soccer Camp. I hereby accept and assume these risks and dangers.
By signing and/or acknowledging acceptance of this Agreement you exempt and relieve Sole Soccer Camp, Jonathan de Guzman and Mississauga Stadium from liability for personal injury, property damage and/or wrongful death caused by negligence or any other cause.
I fully understand that scouts may be present during the camp. I also fully understand that my child/children may be filmed while participating in the camp and that such footage belongs to Sole Soccer Camp and can be used and/or posted, in its sole discretion, on Sole Soccer’s website and/or other social media.
In consideration of Sole Soccer Camp, Jonathan de Guzman and Mississauga Stadium permitting my child/children to participate in its activities and sports, permitting my child/children the use of its equipment and permitting my child/children the use of its facilities, I further agree as follows:
1. TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against Sole Soccer Camp, Jonathan de Guzman, Mississauga Stadium and its directors, officers, employees, agents, and representatives.
2. TO RELEASE Sole Soccer Camp, Jonathan de Guzman, Mississauga Stadium and its directors, officers, employees, agents, and representatives from any and all liability for any loss, damage, injury, or expense that my child/children may suffer or as a result of his/her participation in activities and sports offered by the above mentioned, due to any cause whatsoever, INCLUDING NEGLIGENCE, BREACH OF CONTRACT, BREACH OF STATUTORY DUTY OF CARE, AND/OR BREACH OF THE OCCUPIER’S LIABILITY ACT, R.S.O. 1990, C.O.2 on the part of the above mentioned persons/entities and its directors, officers, employees, agents, and representatives.
3. TO HOLD HARMLESS AND INDEMNIFY Sole Soccer Camp, Jonathan de Guzman, Mississauga Stadium, and its directors, officers, employees, agents, and representatives from any and all liability for any property damage or personal injury to any third party, resulting from my child/children’s activities and participation in the activities offered by or associated with Sole Soccer Camp.
4. That this Agreement shall be effecting and binding upon my heirs, next of kin, executors, administrators and assigns, in the event of my child/children’s death.
5. I have read and understood this Agreement prior to signing and/or acknowledging acceptance. I am aware that by signing and/or acknowledging acceptance to this Agreement, I am waiving certain legal rights which I or my heirs, next of kin, executors, administrators and assigns may have against Sole Soccer Camp and its directors, officers, employees, agents, and representatives.
